We provide in-home functional cognition assessments. We go beyond standard screenings to give families and providers a deeper understanding of a client’s capacity to manage daily life without support.

The Problem with "Pen and Paper" Tests

Standard cognitive screenings performed in a doctor's office only tell half the story. A patient might be able to count backward or draw a clock, yet still leave the stove on or mix up medications.

The Functional Advantage

Our occupational therapy approach looks at functional cognition—the thinking skills required for doing. By assessing clients in their home environment, we can:

• Identify subtle safety risks that clinic-based tests overlook.
• Determine the exact level of support needed for independent living.

Maximize independence and reduce fall risks with  a comprehensive Home Environment Evaluations. We will assess your living space to identify barriers to safety. We go beyond simple checklists, providing a personalized roadmap that includes specific recommendations for adaptive equipment (DME) and structural home modifications. Whether you need bathroom safety solutions, ramp assessments, or kitchen accessibility updates, our goal is to create a safe environment that supports long-term aging in place.

We offer restorative services designed for long-term wellness, not acute recovery. These non-skilled sessions are crucial for maintaining functional gains by targeting the physical skills needed for everyday living. Our programs emphasize:

Strengthening: Improving the power needed for transfers and navigating steps.

Endurance Training: Extending the ability to walk longer distances for errands and community access.

Stretching & Mobility: Enhancing flexibility needed to safely get in and out of the car or reach items in the home.

These sessions are key to enhancing home accessibility and ensuring the physical capacity to enjoy life in and out of the house well into retirement.

Functional Cognition Assessments: $500 for a general assessment within the home using comprehensive standardized testing and home safety evaluation.

Functional Cognition Screen: $350 for functional cognition screen and home evaluation.

Home Safety Evaluation: $250 for a general home safety evaluation 

Restorative Services: $200 for first evaluation visit and $150 per regular session
